Primary biliary cholangitis is a chronic autoimmune condition in which the immune system slowly damages the small bile ducts inside the liver. This causes bile to build up and, over years, can lead to liver inflammation and scarring (cirrhosis). It mainly affects women and is often picked up on blood tests before symptoms appear. Common symptoms are fatigue and itching, and later there may be signs of liver problems. It is diagnosed with blood tests (including specific antibodies and liver tests) and sometimes imaging or a biopsy. Progression varies greatly, and many people do well, especially when treated early.

How it is treated

Treatment aims to slow the damage and relieve symptoms. A specific medicine that improves bile flow is usually started to slow progression and protect the liver, with a further option added if the response is not adequate. Itching and fatigue are treated in their own right, and bone health and vitamin levels are monitored, as these can be affected. People are followed up by a liver specialist, and a minority with advanced disease may be considered for liver transplantation. Avoiding alcohol and keeping to a healthy weight protect the liver.
