When to use

Use to assess a patient's risk of developing pressure ulcers and to guide preventive care. Note: a LOWER score means HIGHER risk.

It identifies at-risk patients so prevention (repositioning, surfaces, skin care) can start early.

Advice

Lower scores = higher risk. Combine with skin inspection and local pressure-ulcer prevention policy; reassess regularly.
