When to use

Use in a patient with LBBB (or paced rhythm) and suspected acute MI; a score ≥3 is specific for acute MI.

Why use

It helps detect MI where the usual ST criteria are obscured by LBBB.

Advice

A score ≥3 is specific (but not sensitive) for acute MI in LBBB. The modified Smith-Sgarbossa criteria improve sensitivity.
